Canceling or Missing Appointments

THIS NOTICE DESCRIBES OUR POLICIES FOR CANCELING OR MISSING APPOINTMENTS. PLEASE REVIEW IT CAREFULLY.

You will be charged $40.00 for missed appointments not canceled at least 24 hours prior to your scheduled appointment. This charge is solely your responsibility and is not covered/paid by your insurance carrier. You may cancel or reschedule an appointment by calling (720) 279-1388. Appointments cannot be canceled online or by email.
